Breakthough's New Psychiatric Program Open for Referrals


Breakthrough has begun their new psychiatric program. Referrals can be made for this service or housing by downloading the form from Google Drive here. The form can be used to request both housing and psychiatric care.

The program is currently funded through September and will be seeking additional funding. Currently, the program works with Hunter Health for appointments.

Appointments are typically available within a week of requesting service. Services are at no cost to the individual. Insurance is not required.

Medication is not a requirement of the program; a person can decline medication management and still be seen for their mental health care.

A person is eligible for this program if they need mental health services and do not currently have a provider. There are no income or housing status requirements.


A person might want to participate in the program if they:

  1. Are experiencing mental illness

  2. Need an updated diagnosis for a disability claim

  3. Need help with a disability application

  4. Would like to explore medication options for managing symptoms
